What is a sinus lift?

A sinus lift is an oral surgery procedure that involves the raising of the level of the sinus in order to gain the space and bone material for the implant installation.

Why is the sinus lift necessary?

In the case of molar or pre-molar loss, the bone in this area becomes gradually disintegrated, i.e. the resorption occurs. Over time, the sinus cavity expands, leaving the unsatisfactory bone level which at that moment forms the barrier between the sinus and oral cavity. By lifting the sinus, we create a favourable situation for implant installation since the connection between the implant and the sinus cavity is contraindicated.

What does the procedure look like?

The procedure is conducted in local anaesthesia and is completely painless for the patient. A surgeon must make an incision and then lift the mucous membrane and access the bone wall of the sinus. After that, a “window” is made on the wall of the sinus, after which the mucous membrane of the sinus is carefully separated and the space between the floor and the mucous membrane of the sinus is created. The created space is filled with artificial bone, and the opening is covered with the membrane, after which the wound is sewn.

Can the implants be installed in the same procedure?

Yes. Implants can be momentarily installed after the sinus lift. Also, the installation can be postponed for a period of four to six months. Whether implantation will be delayed or done in the same procedure depends on the complexity of the case and the surgeon’s assessment.

What is needed before the start of the operation?

The patient must make a 3D image upon which the surgeon performs the analysis and plans the surgical procedure.

Are there any contraindications?

The performance of the sinus lift is contraindicated if the patient suffers from a chronic or an acute sinus inflammation, as well as if the patients has cysts that are localized in the sinus. Patients with poor general health conditions and those who suffer from systematic diseases are also not candidates for such surgical procedure.

What to expect after the intervention?

After an intervention, it is possible to expect the swelling and moderate pain in the area of ​​intervention. The pain can be alleviated by using the cold compresses on the spot for five minutes, with half-hour breaks. If necessary, pain killers can be used. The antibiotic is administered to the patient as a preventative measure and they are advised to avoid excessive physical effort.

How does our face become swollen due to our teeth?

Neglected and non-treated teeth can often lead to the appearance of swollen tissue around the teeth. The cause of swelling is an infection spreading through the inside of the tooth (“nerve”) to the tissue around the tip of the root of the tooth.

How do the teeth which cause swelling look like?

In the most cases those are the teeth showing a substantial visible destruction caused by the tooth decay or the teeth whose old fillings have damaged edges, accompanied by the secondary tooth decay around the filling.

Does the swelling appear only if the tooth is rotten?

No. The swelling and other complications may occur as a result of an inadequate endodontic therapy, with the emphasis especially on the root canals which are not completely filled and on the presence of bacteria in the canal after therapy.

Can the swelling occur because of the presence of periodontal pockets?

Definitely. It is well-known that periodontal pockets, together with the gum inflammation can cause painful swelling of the gums and tissue around the tooth.

What to do if we get swollen?

You need to contact your nearest dentist as soon as possible. Cold compress placed on the cheek for five minutes with a break of twenty minutes can slightly alleviate the problems.

What is the therapy?

First of all, it is necessary to determine the cause of the swelling. Often, the drainage of the swelling through the tooth is performed, and in cases of massive swelling, the incision (cut) on the swollen area is done in order to perform drainage of inflammatory content and decompression of the tissue. Placing the rubber drain in the incision is quite beneficial. The tooth which caused the swelling often needs to be extracted. The dentist will most likely prescribe antibiotics.

What happens if I prolong going to the dentist?

The swelling is expanding gradually. When inflammatory content, due to anatomical structures, cannot spread any further, a fistula occurs, i.e. a natural opening through which the inflammatory content will flow out in the outer environment. Naturally, the problem will not be resolved by itself, and it is possible that the complications which are potentially threatening for the patient’s life occur.

PRF therapy

What is PRF?

PRF is an abbreviation for the term “platelet-rich fibrin”, which in plain words mean fibrin rich in platelets. It is primarily used in oral, aesthetic surgery, cosmetics and mesotherapy.

What can PRF therapy be used for?

PRF method is used in dentistry with the aim to enhance healing of the bones and soft tissues. It is recommended for the patients who have been indicated to undergo surgical therapy because it makes the intervention faster, safer and painless.

PRF therapy is used for:

• Periodontal intervention (flap surgery).

• Therapy for gingival recession (gum recession) – PRF membrane is used to cover the bare neck of the tooth.

What to do in case of knocked out tooth?

After the injury, it is crucial to stay calm, take the knocked out tooth and go to the nearest dentist as soon as possible.

What is the most usual cause of knocked out tooth?

The tooth is knocked out because of the fall or the hit. These injuries usually happen to children, quite rarely to the adults.

When should one go and see the dentist?

It is desirable to see the dentist in the shortest possible time. If you go to the dentist immediately after the injury, within half an hour, the result of putting the tooth back in its place will be more certain and more favourable.

How to get the tooth to the clinic?

The knocked out tooth must be dealt with special care. It is best if you take the tooth by its crown and avoid the contact with the root in order not to contaminate it with the impurities. If possible, it would be best to place the tooth in the physiological solution or in milk and take it to the nearest dental clinic. Also, the tooth can be placed under the tongue where the saliva will keep it moist. Priority should be given to physiological solution or milk.

How does the putting the tooth back look like?

First of all, it is important to clean the wound, and to rinse the tooth in the physiological solution. Then the tooth is placed back in its original position and it is immobilised, connected with the neighbouring teeth in order not to move. The immobilisation lasts approximately for a month for which time it is suggested that the patient should eat soft food.

What is the result of the therapy?

If little time has passed from the moment when the tooth was returned to alveolus, we can expect a positive result. After the tooth has become firmly fixed in the bone, it is necessary to perform endodontic treatment because the nutrient absorption in the tooth has been interrupted and the pulp gets in the state of necrosis. In smaller numbers of cases the tooth will be vital (with restored nerve) after it has been placed in its original place. Why do we grind our teeth?

What is bruxism?

Bruxism is a parafunctional activity, which involves clenching and teeth grinding. It happens on a subconscious level and is not related to sex and age.

What causes bruxism?

There is daily bruxism, when the patient consciously clenches his/her teeth, exposing them to non-physiological forces. However, night bruxism is much more frequent. It occurs as a result of muscle hyperfunction, emotional stress, and anxiety. This is most common during sleep.

What are the consequences of the bruxism?

The consequence of teeth grinding causes the overload of the tooth support device, as well as the jaw joint. The consequences include the following: the teeth are consumed (tooth abrasion, “teeth eaten to the nerve”), headache, the fillings get cracked or even broken due to excessive pressure, as well as pain when opening / closing the mouth, muscle ache.

How is bruxism treated?

Therapy is symptomatic, removing stress, if possible. The next measure involves the creation of an individual splint (the so-called night guard), in order to reduce abrasion, protect the dental tissue and reduce muscle pain. Splint is worn during sleep, and is made on the basis of a tooth print.In case of stronger consumption of the tooth, prosthetic therapy is necessary to reconstruct the correct occlusion, and often, due to the great consumption of the tooth, endodontic (tooth treatment) is also necessary.

Why does a tooth get darker?

The most common reasons why a tooth can get darker are endodontic therapy, better known as “nerve extraction” and tooth trauma. In both cases, discolouration is caused by the tooth blood vessels important for the nutrients absorption get severed. A tooth can change its colour due to the necrosis or pulp gangrene.

How long after the nerve extraction will the tooth become darker?

There is no an exact period after which we can say the discolouration of a tooth will occur. Most often the changes occur after a few years since the endodontic therapy or tooth trauma.

Can the colour of the tooth after endodontic therapy be affected in some way?

Change in color, i.e. the tooth darkening  certainly occurs and it is different from one person to another. Adequate endodontic treatment and compliance with protocols in therapy can affect the reduction of tooth darkening.

How to solve the problem of a dark tooth?

There are several possibilities. First of all, the patient is advised to whiten their teeth, whereby the bleaching agent is placed inside the tooth as well. If the bleaching does not yield the desired results, the dark surface of the teeth can be covered with a composite or ceramic facet. In addition to the above mentioned therapeutic possibilities, the tooth can be remediated with a metal ceramic or non-metallic crown.

My tooth is darker but yet there has been no nerve extraction

In this case, the issue is most likely a disorder in the composition of the tooth enamel or dentin (a tooth layer under the tooth enamel). These changes are most commonly found in the stage of tooth development.

Are the darker teeth more susceptible to bleeding?

U ovom slučaju najverovatnije se radi o poremećaju u sastavu gleđi ili dentina (sloj zuba ispod gleđi). Ovakve promene najčešće nastaju u stadijumu razvitka zuba.

Da li su tamniji zubi podložniji kvarenju?

No. regardless of the shade of the tooth colour, each tooth will be protected from the tooth decay by maintaining the oral hygiene regularly.

What is tooth extraction?

Tooth extraction is an intervention used to remove the tooth from its position in the jawbone.

What is the most common cause for tooth extraction?

In vast majority of cases, the tooth is so affected by caries that the only solution is tooth extraction. Such teeth are usually contraindicated for any recovery with a seal or crown.

How to determine that a tooth is to be extracted?

Clinical examination and X-ray image analysis make it easy to determine the presence of caries lesion which has affected the tooth root deep beneath the gum. Such teeth should be extracted as soon as possible.

Is great caries the only sign that the tooth should be extracted?

Of course not. Beside caries, the tooth can be affected by infection or cyst, which is localized at the top of the tooth root. If it is determined that the infection or the cyst cannot be cured with other methods, the tooth should be extracted. Otherwise, the situation can get worse and the infection can spread to adjacent tissue.

Are fractured teeth necessarily extracted?

If the fracture is localized beneath the gum and affects the root, it is usually necessary to extract the fractured tooth.

Are healthy teeth sometimes extracted?

Yes. The only reason for extracting healthy teeth is the need for adequate orthodontic therapy. The tooth is extracted in order to create space for regular positioning of other teeth. The extraction is done upon written request by the orthodontist who performs the orthodontic therapy.

What can you expect after tooth extraction?

Several days after the extraction, slight unease and minimal pain can be felt. If the symptoms include marked swelling, pain, or something similar, it is necessary to contact your dentist.

How much time is needed for the wound to heal after the extraction?

It takes approximately a month and a half for the wound to heal properly. After this period, the gum completely covers the space of the extracted tooth.

Why do we have problems with wisdom teeth?

Wisdom teeth or (late-blooming molars) are the last teeth to grow, in the period from 17 to 25 years of age. This is the reason why sometimes there is no space in the jaw so the wisdom teeth cannot grow, that is, they grow only half-way or remain “trapped” in the bone beneath the gum.

What kind of problems can we have with half-grown wisdom teeth?

Half-grown wisdom tooth usually does not have adequate position next to the neighbouring tooth, which can create periodontal pocket between the wisdom tooth and the neighbouring tooth. Furthermore, with half-grown wisdom teeth, there is greater possibility of caries on places which are unavailable for cleaning due to inadequate teeth position.

Can ungrown wisdom tooth make problems?

Of course! In time, cyst can occur in the bone around the ungrown tooth. The cyst gradually widens. If inflammation occurs, pain will be the first sign that will lead you to your dentist.

Can the wisdom tooth damage the tooth in front of it?

Yes. With its inadequate position and growth in inadequate direction, the wisdom tooth can damage the root of the second molar, i.e. the tooth in front of it.

Is wisdom tooth extraction complicated procedure?

Half-grown and ungrown wisdom teeth are extracted with a small surgical intervention which lasts for approximately twenty minutes.

Is surgical extraction necessary with half-grown wisdom teeth?

Yes, it usually is. Surgical extraction is necessary in order to approach the wisdom tooth by elevating gums, and to release the part of the bone which does not allow tooth extraction.

Is the intervention painful?

Surgical wisdom tooth extraction is performed with local anesthesia and it is absolutely painless.

What can you expect after wisdom tooth extraction?

Mild swelling can occur after the intervention. It is the most prominent on the second day after the extraction. Slight pain at the wound area and uneasy feeling are also present a few hours after the operation.

What are interdental toothbrushes?

Interdental toothbrushes are specially constructed toothbrushes, intended for cleaning interdental space, i.e. the space between two neighbouring teeth.

Why is it necessary to use interdental toothbrushes?

It is known that after food consumption, during the day, a thin layer called the plaque is formed against teeth and gums. Since the plaque contains millions of bacteria, the very presence of it gradually leads to gum inflammation, which can in time cause periodontitis and other teeth and gum illnesses. The plaque can easily be cleaned using the standard toothbrush, but the plaque between teeth can only and completely be removed with interdental toothbrushes.

How and when should I use the interdental toothbrush?

Ideally, it should be used three times a day. Interdental toothbrush is used after tooth brushing while there is still toothpaste in your mouth. The space between teeth is cleaned with gentle movements. Once a single interdental space is cleaned, the brush should be rinsed with water, and then moved on to the next space.

What if my gums bleed?

Gums bleed when they are inflamed. In order to stop the inflammation, regular cleaning is necessary so the bleeding should be ignored. Naturally, do not forget that brush movements should be gentle because if we apply too sharp and rough movements, the bleeding will occur as a result of gum injury.

What brush size should be chosen?

There are various sizes of interdental brushes, depending on whether teeth are too compact or not. The brush size can be determined precisely by interdental space width meter.

When should the interdental toothbrush be replaced with a new one?

After certain period of usage, the interdental fibers get worn out. Such toothbrush cannot clean interdental space adequately so it is necessary to get a new one. By using old and shabby toothbrush, we can hurt the interdental papilla, i.e. the part of the gum placed between two neighbouring teeth.

Why is regular mouth hygiene important?

Regular teeth brushing eliminates food residues and soft deposits which are created after every meal. Regular teeth maintenance prevents occurrence and development of caries, as well as gum illnesses.

How do soft deposits affect gum health?

Soft food deposits contain thousands of different bacteria which destroy gum and cause gum inflammation. First, gum swelling occurs along with forming of space between teeth and gum (gingival pocket). Such environment is ideal for further bacteria accumulation, which leads to more severe gum inflammation and creation of periodontal pocket.

What is periodontal pocket?

With gingivitis advancement, i.e. gum inflammation, pathological space is created, limited by tooth surface and gum edge. This space is called periodontal pocket and it is the main sign of periodontitis. With periodontitis advancement and pocket widening, tooth stability and support is disturbed, which leads to tooth wiggling or even falling out.

How to stop gingivitis occurrence?

The only prevention of this illness is regular and appropriate oral hygiene, which means tooth brushing at least in the morning and in the evening, along with using interdental toothbrush.

What if my gums bleed during tooth brushing?

The most common cause of gum bleeding during tooth brushing is gum inflammation. Regardless of bleeding, teeth should be brushed regularly. As mouth hygiene goes to higher level, so the inflammation will disappear, as well as bleeding.

What does gingivitis and periodontitis therapy look like?

Apart from oral hygiene maintenance, cleansing of dental calculus and soft deposits can be performed, as well as curettage of periodontal pockets. If deeper pockets occur, surgical treatment is applied.

When should the dentist be contacted?

If you have gum bleeding and unpleasant breath for a longer time, contact your dentist as soon as possible.
